The Journal of Patient Safety published a study that puts preventable errors third on the list of causes of death in America, behind only heart disease and cancer. Patient safety experts say that no one knows the actual number of deaths as a result of preventable mistakes. However, they agree that the new estimate, developed by John T. James, came as a result of credible methods and findings. James is a toxicologist at NASA’s space center in Houston and leads the advocacy organization, Patient Safety America. He says that preventable hospital errors that contribute to a lack of patient safety and patient deaths is an issue that requires immediate public attention. James wrote that it may be “time for a national patient bill of rights for hospitalized patients,” in his study.
